Church Basket Request Form

FPC church members may submit the names of individuals or families that they would like to receive a Christmas Food basket. These are “Food Only” baskets. Children in these families will not be included on the Angel Tree for gifts.

We ask that the church member identifying this “food only” basket be the one to pack and deliver the baskets on Saturday, December 9th. Assembly of all food baskets will take place in the church Fellowship Hall on December 9th beginning at 9am. We are asking that you then take your food basket out for delivery.

Please complete this form by Sunday, November 19tj and return to the church office.
